Ozempic Dosage Chart 2026: Weekly Schedule, Pens, Missed Doses and Unit Conversions

Each step is held a minimum of 4 weeks; longer holds are common if side effects linger or glucose and weight goals are already met. Increases are a prescriber decision, not automatic. Inject once weekly, any time of day, with or without food, rotating abdomen, thigh and upper arm.

Ozempic dose for weight loss vs diabetes

The schedule is identical; the target differs. For type 2 diabetes, many patients reach their A1c goal at 0.5 mg or 1 mg and stop there. For weight loss (off-label), appetite suppression scales with dose and most of the effect appears at 1-2 mg. Wegovy — the same molecule labelled for weight — adds a 1.7 mg step and a 2.4 mg maximum. Expected loss by dose is covered in how much weight you lose on Ozempic.

2 mg—80 units (0.80 mL)40 units

Units above assume a U-100 insulin syringe (100 units = 1 mL). Always use the concentration printed on your vial — compounded concentrations vary by pharmacy. Full method in the reconstitution and units guide. Switching to tirzepatide follows a different rule: see Ozempic to Mounjaro conversion.

Missed dose and schedule changes

  • Missed by ≤5 days: inject as soon as remembered; keep your usual day.
  • Missed by >5 days: skip; inject on the next scheduled day. Never two doses within 48 hours.
  • Missed 2+ weeks: ask your prescriber about restarting one step lower to avoid nausea.
  • Changing injection day: allowed any time as long as 48 hours have passed since the last dose.

Side effects by dose step

Nausea, reflux and constipation peak in the first 1-2 weeks after each increase and fade as the body adapts. If a step is rough, holding 4-8 weeks longer costs nothing clinically. Practical fixes are in GLP-1 nausea relief and Ozempic side effects.

Cost by dose

Brand Ozempic is priced per pen, not per dose: ~$998 retail, $349/month at any dose through NovoCare or Costco, $25 with covered commercial insurance. Compounded semaglutide is $119-$199/month flat at most providers regardless of dose — see the GLP-1 Price Index.

Semaglutide with a provider who manages every dose step — from $119/month.
