Does Mounjaro Cause Erectile Dysfunction? What Tirzepatide Does to ED
ED is not a tirzepatide side effect — but a temporary dip in the first weeks is real and explainable. What the trials show, why most men improve by month 3-6, what to do during titration, and when a dip is a warning sign.

Short answer
Mounjaro (tirzepatide) does not cause erectile dysfunction as a drug effect. ED does not appear in the prescribing information, and the phase 3 trials showed no signal versus placebo. What many men do experience is a temporary dip in the first 4-8 weeks — driven by the calorie deficit, fatigue, dehydration and nausea of titration — followed by improvement as weight comes off. For men with obesity-related ED, tirzepatide is more likely to fix the problem than cause it.
What the evidence actually says
| Source | Finding |
|---|---|
| Mounjaro / Zepbound label | ED and sexual dysfunction are not listed adverse reactions |
| SURPASS (diabetes) & SURMOUNT (obesity) trials | No increase in sexual-function adverse events versus placebo |
| Weight-loss and ED literature | 10-15% weight loss improves IIEF erectile scores in 40-60% of men with obesity-related ED |
| Testosterone studies | Significant fat loss raises total testosterone 15-30% via reduced aromatase |
| Case reports / forums | Transient reduced libido or erections during titration; resolves with dose stabilisation |
The broader GLP-1 and sexual-health picture — including semaglutide — is in GLP-1 and erectile dysfunction and GLP-1 and libido.

Why some men notice a dip in the first weeks
- Calorie deficit and fatigue. Appetite drops hard at 2.5-5 mg. Eating 800-1,200 calories with little protein leaves no energy for anything, including sex. See GLP-1 fatigue.
- Dehydration. Nausea plus drinking less reduces blood volume; erections depend on blood flow. Read GLP-1 dehydration.
- Poor sleep. GI side effects and reflux during titration fragment sleep, which suppresses morning testosterone.
- Lower libido, not lower function. GLP-1s blunt reward signalling broadly — food, alcohol, and for some men sexual desire — in the early weeks. This usually normalises.
- Mood. Feeling unwell for a month affects confidence and arousal; this is psychological, not vascular.
Why most men improve by month 3-6
Obesity causes ED through three routes: visceral fat converts testosterone to estrogen, chronic inflammation damages the endothelium that lines penile blood vessels, and insulin resistance impairs nitric oxide. Tirzepatide reverses all three as weight falls — at 20% weight loss (the SURMOUNT average at 15 mg) the vascular effect is substantial. The timeline mirrors weight loss: testosterone shifts in month 1-3, endothelial function in month 3-6, with the largest gains after 10-15% loss.
What to do during the dip
- Eat enough protein — 1.2-1.6 g/kg/day; it preserves energy and muscle. See the protein guide.
- Hydrate deliberately — 2.5-3 L/day with electrolytes if nauseous.
- Slow the titration — staying at 5 mg an extra 4 weeks costs little and often resolves side effects.
- Resistance training 2-3×/week — supports testosterone and prevents muscle loss.
- PDE5 inhibitors are fine — tadalafil is less affected by delayed gastric emptying than sildenafil.
When the dip is a warning sign
See a clinician if ED persists past 8-12 weeks on a stable dose, appears suddenly with no GI side effects, or you have diabetes, hypertension or high cholesterol. New ED is frequently the earliest sign of cardiovascular disease and deserves a testosterone panel, A1c, lipid profile and blood pressure check regardless of medication. Do not stop tirzepatide on your own.
Mounjaro vs Zepbound vs semaglutide for ED
Mounjaro and Zepbound are the same molecule; the effect on sexual function is identical. Compared with semaglutide (Wegovy/Ozempic), tirzepatide produces more weight loss (~20% vs ~15%), so the downstream benefit on obesity-related ED is expected to be larger, though no head-to-head sexual-function trial exists. Neither drug directly affects erectile tissue.
