GLP-1 and L-Carnitine: Can It Boost Weight Loss?

L-carnitine is marketed as a fat burner that transports fatty acids into mitochondria. But does it actually enhance GLP-1 weight loss? Here's what the science says.

What Is L-Carnitine?

L-carnitine is an amino acid derivative that transports long-chain fatty acids into mitochondria, where they're burned for energy. Without sufficient carnitine, fat cannot be efficiently used as fuel. Your body produces it from lysine and methionine, but supplementation may increase levels beyond baseline.

How L-Carnitine May Complement GLP-1s

  • Fat oxidation: May increase the percentage of fat burned vs muscle during weight loss
  • Energy production: Helps combat GLP-1 fatigue by improving mitochondrial function
  • Muscle preservation: May reduce muscle breakdown during rapid weight loss
  • Exercise performance: Improves recovery and reduces muscle damage from workouts
  • Insulin sensitivity: Some studies show improved glucose uptake

Forms of L-Carnitine

FormBest ForDoseCost/month
Acetyl-L-carnitine (ALCAR)Brain health, energy, fatigue500-1,500mg$15-25
L-carnitine L-tartrateExercise performance, muscle1,000-2,000mg$15-20
Propionyl-L-carnitineCardiovascular health500-1,000mg$20-30
Glycine propionyl-L-carnitine (GPLC)Exercise + blood flow1,000-3,000mg$25-35
Plain L-carnitineGeneral, budget1,000-2,000mg$10-15

What Studies Show

A 2020 meta-analysis of 9 studies found L-carnitine supplementation produced an average of 1.33kg (2.9 lbs) additional weight loss compared to placebo. A 2022 study found that L-carnitine combined with aerobic exercise increased fat oxidation by 15%. However, a 2020 JAMA study questioned carnitine's effectiveness, finding no significant weight loss in a well-controlled trial.

For GLP-1 users specifically, no direct studies exist. The theoretical benefit is that L-carnitine may help preserve muscle and support energy during the rapid weight loss that GLP-1s produce.

How to Take L-Carnitine on GLP-1s

  • Start with 500mg ALCAR with breakfast
  • Increase to 1,000-1,500mg after 1 week if tolerated
  • Take 30-60 minutes before exercise for performance benefits
  • Take with food to reduce nausea
  • Avoid taking after 3 PM — may cause insomnia
  • Cycle: 8 weeks on, 2 weeks off to maintain effectiveness
