What Is CagriSema?

CagriSema is an investigational once-weekly injectable medication that combines two separate drugs into a single injection:

• Semaglutide — the GLP-1 receptor agonist found in Wegovy and Ozempic
• Cagrilintide — a long-acting amylin analogue that targets amylin and calcitonin receptors

Amylin is a hormone co-secreted with insulin from the pancreas. • REDEFINE-4 (head-to-head vs tirzepatide): CagriSema produced 23% weight loss vs tirzepatide's 25.5% — CagriSema lost the head-to-head but still showed strong results.
• REDEFINE-2: Results in patients with type 2 diabetes and obesity.
• REDEFINE-3: Results in patients with cardiovascular disease and obesity.

While CagriSema missed Novo Nordisk's internal target of 25% weight loss, it still represents a significant improvement over Wegovy alone (15%) and approaches tirzepatide's efficacy (22.5%).

When Will CagriSema Be Available?

Novo Nordisk submitted its New Drug Application (NDA) for CagriSema in December 2025. The FDA is expected to make a decision in late 2026 or early 2027. If approved, CagriSema would be:

• Available in a dual-chambered pen device (the two drugs cannot be pre-mixed)

• Retatrutide (Eli Lilly) — Triple agonist targeting GLP-1, GIP, and glucagon. 28.7% weight loss in Phase 3. BLA planned Q1 2027.
• Amycretin (Novo Nordisk) — Single molecule targeting both amylin and GLP-1. 22% weight loss in Phase 1b/2a at 36 weeks.
